We are now accepting new patients.

How to Enrol

You can enroll by filing out the enrolment forms below along with the medical questionnaire and portal registration form. Email these forms to our reception at reception@aucklandfamily.co.nz with your doctor of choice, along with your Passport or Birth Certificate. If you are not a New Zealand resident please send us a copy of your work visa/ proof of eligibility. Please see attached below on how to enrol and eligibility.

ENROLMENT INFORMATION

New Patient Fees

New Patients are required to have a 30 minute initial appointment. This appointment is charged at $160. The doctor will thoroughly discuss your past history and get to know you as a patient. Please call reception to make your first appointment, DO NOT BOOK ONLINE.
