Appointment Requirements

A 30-minute travel consultation is required for all travel vaccine appointments.

Appointments must be booked at least 5-8 weeks before your travel date.

Travel forms must be completed and submitted at least 3 days before you appointment date.
During your consultation
During your appointment, our GP and nursing team will:

Review your travel itinerary and health needs.

Provide recommended vaccinations and medications.

Offer personalised travel health advice.

To help us prepare for your visit, please complete our Pre-Travel Checklist Form and email it to us: reception@aucklandfamily.co.nz at least 3 days before your appointment or the earlier the better.

Complete one form per traveller and ensure all details are accurate.
Important Information

Some vaccines require multiple doses over time.

Not all vaccines may be available on the day.

Travel vaccines are not funded and costs may vary.
